Washington's My Health My Data Act (MHMDA)
1. What Is Consumer Health Data
“Consumer health data” means personal information that identifies a consumer's past, present, or future physical or mental health status. In the context of any Jeannie Oliver Wellness, LLC Program, this may include information you share with us such as:
Dietary habits, food sensitivities or allergies, and eating patterns
Weight, health goals, and self-reported symptoms
Supplement or medication use you choose to disclose
Responses to intake forms, surveys, or check-ins related to the Program
2. Health Data We Collect and Why
We collect the categories of consumer health data listed above directly from you when you register for the Program, complete an intake form, or communicate with us during the Program (e.g., email, group chat within the Practice Better client portal). We use this information only to: (a) provide and personalize the Program to you, (b) respond to your questions, and (c) improve our Program materials.
3. We Do Not Sell Your Health Data
We do not sell consumer health data. We do not use geofencing around any healthcare facility, mental health facility, or reproductive/gender-affirming care facility to identify, track, or send communications to consumers.
4. Your Consent
We will not collect consumer health data from you until you affirmatively opt in via the form sent to you upon joining the Practice Better Client Portal, separately from your agreement to any Program Agreement with Jeannie Oliver Wellness, LLC.
If we ever want to use your consumer health data for a purpose not described above, we will ask for your separate, additional consent first.
